DUTIES AND RESPONSIBILITIES:
Establish a trusting non-judgemental rapport with clients.
Assess each client, determine next movement and goals for each. Refer to transition house, other resources. Complete follow up once exited clients.
Ensure clients will be safe and/or will have increased protection from outside or inside influences
Ensure that residents basic needs are met during their stay, such as shelter, food, & basic hygiene
Assist clients with emergency needs at intake, following processes and policy.
Design and implement a referral system for client success. Such as mental health, Health, Housing, Elders & Counselling, Training, and placement.
Respond to appropriately to crisis situations as outline in process and policy.
Ensure data is entered for each client. Report deficiencies to Manager. Guide support workers to ensure data is entered.
