The Home Care Support Worker is to provide preventative health care initiatives and health promotion programming with clients. The Home Care Support Worker will work with a multi-disciplinary team of Health Care professionals and support clients in their home to assist with daily living requirements. The Support Worker will provide transportation for clients to participate in community services which include, but not limited to pre- and post-natal clinics, school health programs, chronic disease clinics, immunization programs, and communicable disease surveillance and treatment. The services may be provided at the OAFN Health Centre, community buildings, school, or in clients home. Ability to work independently and highly organized; Excellent written and oral communication skills; Must have First Aide & CPR/AED, Safe Food Handling; Valid Saskatchewan driver's license and a reliable vehicle; Criminal record check and vulnerable sector; Mental Health First Aide and ASIST training would be an asset.
