GENERAL ACCOUNTABILITY:
Reporting to the Manager, Population Health, responsible for the Lung Screening and Prevention (LSP) Program, the Smoking Cessation Case Coordinator is responsible for assessing program participants readiness for smoking cessation, identifying their needs, and providing client-centred support, counseling, and referrals to appropriate services. The position also conducts follow-up assessments, tracks progress, and celebrates client achievements to encourage long-term cessation success.
This role actively contributes to the development, implementation, and evaluation of program strategies and projects aimed at improving service delivery within the Lung Screening and Prevention Program. This includes supporting training and education initiatives to enhance service coordination, quality, and effectiveness. The position works collaboratively with internal and external stakeholders to ensure the program's objectives are met.
QUALIFICATIONS:
Education:
- University Degree in a health or human services field.
Experience:
- Two (2) years experience counselling or providing interventions to clients in a mental health or addictions setting.
- Experience in managing a client or patient case load, including assessing needs, making plans and following up on progress.
