Duties include:
- Quality of Life (QOL) Coordinator will be the first point of contact for families and service providers seeking Quality of Life services.
- Coordinates all quality of life programming (FunAbilities, Summer Fun, Acquired Brain Injury Life Enrichment Program) including accepting applications, conducting observations, developing and facilitating programming, seeking funding, collecting payments and invoicing.
- Schedules and supervises Quality of Life program staff.
- Provides direct support to individuals experiencing disability in Quality of Life programming including evening and weekends and during the months of July and August.
- Conducts client intake which may include: assessments, observations and interviews to determine needs, and supports required to successfully participate in programming.
- Prepares information for invoicing; reviews and approves expenses for all programs up to $500.00 and ensures program operates within budget.
- Documents information, including Application for Services, Safety Plans, attendance, Quick Reference Pages, file recordings, and incident reports.
- Collaborates and communicates with community agencies and caregivers to address and respond to gaps in the community for individuals experiencing disability.
Qualifications:
Education:
- Bachelor's Degree in the Human Services field or equivalent experience
- Valid Class 5 Driver's Licence and access to a working/reliable vehicle
- Valid First Aid/CPR/AED Certification or willingness to obtain within three months
- Proficient computer skills and good knowledge of Microsoft suite of programs / skill in using social media platforms
Experience:
- Two years experience in related field
