Description

The One Roof Collaborative is seeking a skilled and values-driven Project Manager to support the planning and implementation of the One Roof Initiative  a transformative, Indigenous-led model of integrated health and social care. Rooted in Indigenous values, cultural safety, and holistic integration, One Roof sets a national precedent in community-based health transformation and will accelerate advancements in the patient medical home (PMH) model of care.

The successful candidate will lead and coordinate key components of this phased integration initiative, supporting operations and collaborative planning between core partners: the Saskatoon Tribal Council, the Saskatchewan Health Authority, the Saskatoon Community Clinic, and other clinical and community organizations that support complex patients in Saskatoons core neighbourhoods, including Sanctum Care Group and the Saskatchewan Infectious Disease Care Network.

Reporting to the One Roof steering committee, the Project Manager will oversee the development, coordination, and implementation of the One Roof workplan, with a special focus on:

Supporting Indigenous-led leadership and community engagement processes.

Advancing culturally safe, integrated service delivery models.

Strengthening the Patient Medical Home (PMH) model in Saskatoons core neighbourhoods.

Linking HIV prevention and care through intersectional coordination.

Facilitating team-based approaches that avoid duplication and optimize existing resources for this population.
