The Community Health Nurse will provide community based, culturally relevant programming and services as well as support initiatives and strategies that aim to improve health outcomes associated with maternal, infant, child, and family health. The focus of care is on health promotion, disease and injury prevention, health protection, health surveillance, population health assessment, and emergency preparedness and response. The nurse will provide optimal client services by collaborating with team members in the planning, implementation, delivery, and evaluation of services.
DUTIES:
    Complete professional competencies and professional knowledge required for community health nursing practices, specifically immunizations, maternal and infant care, communicable disease, and other health issues for those at risk;
    Offer immunization services on and off site in the community for publicly funded and seasonal vaccines;
    Follow Saskatchewan Immunization Guidelines for vaccine storage, handling procedures, documentation methods, inventory, and ordering;
    Assist with the mandatory Tuberculosis Program including administering and evaluating tuberculin skin tests and refer clients as needed per TB protocol;
    Manage notification, treatment, follow up and referral processes related to assigned communicable diseases by current medical directives and according to the Public Health Act;
    Using best practice provide clinical expertise for prenatal/postnatal and infant care;
    Maintain accurate client file and client database records
