Job Description:
Reporting to a Primary Health Care Manager, the Staff Pharmacist is committed to the integration of knowledge and skill in the provision of quality, client-focused pharmacy
services and the ongoing development and optimization of contemporary pharmacist directed patient care activities in a primary health care community multi-disciplinary team
environment within the Saskatchewan Health Authority. As an integral member of the Primary Health Care Team, the incumbent is responsible for chronic disease counselling,
medication-related needs of patients, the provision of drug information to physicians, other health care professionals and the patient, and will serve as a preceptor to other
pharmacists, pharmacy residents, and pharmacy undergraduate students, as required. The incumbent will also be involved in multi-disciplinary education and research. Working
as part of this multi-disciplinary care team, the pharmacist must have the ability to establish effective, trusting working relationships with patients, healthcare teams and other
resource providers. The pharmacist will assist in the planning and development, implementation, coordination, and evaluation of chronic disease programming. The successful
