Job Description: Under the direction of a Program Manager and as part of the interdisciplinary Community Outreach and Support Team, the Assessor Coordinator Sr. provides a complex range of screening, triage, assessment, treatment and case coordination services to a community mental health and addictions population The incumbent provides program consultation and co-ordination, evaluation of clinical interventions, supervision, preceptor students in formal clinical practicum, and provides professional and public education regarding mental health and addiction issues and resources. As a senior practitioner, the incumbent acts as a resource for other professionals for mental health and addictions in the community. The Community Outreach and Support Team provide crisis intervention, support, assessment and follow-up plans to clients in the community who are in crisis or require assistance. The team also assists in the successful stabilization and/or transition of acute patients from hospital/detox to community or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those who present in crisis. Follow-up may include brief monitoring/support, referral to appropriate community organizations/support networks or collaboration with existing supportive organizations.
