Job Summary
Under the direction of a Program Manager and as part of the interdisciplinary Community Outreach and Support Team (CRT), the Community Mental Health Nurse provides a complex range of screening, triage, assessment, treatment and case coordination services to a community mental health and addictions population The incumbent provides program consultation and co-ordination, evaluation of clinical interventions, supervision, preceptor students in formal clinical practicum, and provides professional and public education regarding mental health and addiction issues and resources. The incumbent will provide nursing leadership and clinical expertise in the development and maintenance of community mental health and addiction treatment programs, and ensure nursing practice standards are maintained.

The Community Recovery Team provides crisis intervention, support, assessment and follow-up plans to clients in the community who are in crisis or require assistance. The team also assists in the successful stabilization and/or transition of acute patients from hospital/detox to community or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those who present in crisis.
