Job Description: Provides case management services, which includes assessing, planning, monitoring and evaluating for determining the care needs of clients, collaborative coordination of services and case management process.

Position Summary:
Reporting to the Manager, the incumbent is responsible for the screening and coordination of initial service requests for all programs within Mental Health and Addictions Services in the Saskatchewan Health Authority. The Assessor Coordinator Degree provides screening, triage, assessments for urgency/risk, and coordinates case assignments. The position facilitates crisis response and short-term support for services to clients presenting with a variety of mental health and addictions issues. The Assessor Coordinator Degree provides consultation to Community Agencies and Programs regarding services available in Mental Health and Addiction Services.

Position Duties:
Screening, assessment, and coordination of all initial service requests (written, telephone, walk-in, psychiatry and court ordered referrals.) to MH/AS and determining appropriate service according to client need.
Responding promptly and effectively to initial requests for information and/or services, and planning other appropriate intervention or referral services for requests that are outside the mandate of MH/AS.
