Job Description: Primary responsibility for Chronic Disease Management programs in the area of diabetes, assisting with development/coordination/conduct of inpatient and outpatient programs and clinics. Utilize established criteria to assess and plan each client's selfmanagement care needs, providing appropriate intervention to ensure timely access to care. Provide efficient and effective individual self-management education/counseling for patients/families/caregivers in all aspect of diabetes disease management care and treatment. This will include liaison with families, physicians, dietitians, staff nurses, and community agencies. Coordinate and facilitate group education classes for outpatients in a variety of settings. Promote continuing health professional involvement in patient education through effective communication, development of resource materials and provision of in-service education. Participate in departmental/research activities (program development, planning, quality improvement, and evaluation). Implement program/resource development related to needs assessment, planning, program design, implementation and evaluation.
