GENERAL DESCRIPTION
Under the supervision of the Director of Health, the Community Health Representative (CHR) is responsible for acting in the capacity of a health paraprofessional, assisting the health program to carry the aims and objectives of the nursing program and the health program to achieve community wellness through the holistic healing program utilizing the team approach.
Duties and Responsibilities may include:

    Develop and prepare workshops on current health issues to band membership and school the population.
    Provide emergency first aid as required.
    Home visits for parental consents for immunization, dental or any other services necessary.
    Complete an inventory of medical supplies on a quarterly basis.
    Assist in conducting needs assessments on health issues as requested for evaluation purposes.
    Keep a daily log of activities for incorporation into monthly reports.
    Provide transportation to and from the health center for community members.
    Communicate and collaborate with health center employees and other community program staff.
    Other duties that may be assigned by the Director of health.
