Job Description: Under the direction of the Manager (or designate) and as part of the interdisciplinary team, the Assessor Coordinator - Degree is responsible for the full scope of case management services for rehab patients. This is accomplished through collaboration with the patient, family and other members of the interdisciplinary team. The Assessor Coordinator - Degree liaises with other services in the SHA where patient repatriation needs to occur, in an endeavor to provide the right service at the right time and in the right place. This position reports directly to the Manager for daily clinical supervision.

Education:
- Degree in Human Services or related field (Social Work, Nursing)
- Must have a current practicing registration with Saskatchewan Association of Social Workers or College of Registered Nurses of Saskatchewan
- Must have a valid driver's license. Some assignments may require access to a vehicle for work
Knowledge, Skills & Abilities
- Knowledge of theories, concepts and intervention strategies key for case management practice.
- Cost conscious, quality driven care planning and service delivery
- Knowledge of rehabilitation populations being served
- Demonstrates an understanding of patient confidentiality and the Health Information Protection Act
- Knowledge of advocacy, negotiation, crisis management and conflict resolution
