Job Description: The Mental Health Social Worker will provide client-driven health and social case management assistance to children and youth in the community and those clients attending the Health Centre to achieve safe and reasonable goals that will optimize clients' health and wellness and improve their quality of life. The Mental Health Social Worker works in collaboration with clients, their families and community and government agencies, and works closely with the Mental and Addictions teams. The Mental Health Social Worker also acts as a resource to and supports the development of community and interagency initiatives as they pertain to their mandate. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

P osition Duties:- To provide assessment, counselling, and case management services to children and youth and their families with respect to mental health issues
- To provide child and youth programs and counselling utilizing a family centered approach in partnership with schools and other youth oriented referral agents.
- Performs other case management duties as assigned and within the scope of practice of a Social Worker
