Job Description: Under the direction of the Intensive Services Manager and working within an interdisciplinary mental health and addiction team, the Transition Team Nurse will provide a range of assessment, intervention, navigation services and clinical services to all ages with mental health and addiction issues and support to their families. The incumbent will also serve as an important link between Acute Care Services and the services provided by Community Mental Health and Addictions.

Experience
- 2 years working experience in community mental health and addictions services
Education
- Bachelor of Psychiatric Nursing (BPN)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Psychiatric Nursing diploma is acceptable
- Nursing diploma is acceptable
- Bachelor of Nursing is acceptable
