One Arrow First Nation is looking for a team member to work with clients, families and the community with a focus on preventative health care initiatives and health promotion programming. These services include, but not limited to chronic disease support, treatment, and preventative health strategies. Under the direction of the Health Director, the Home Care RN will also monitor the community for general health, epidemics, and emergencies.
MAIN DUTIES:
-Participate in the service delivery activities of the primary care staff team, ensuring an integrated, collaborative client centered approach.
-Assess clients for physical and psychological needs, their knowledge, their health, learning needs and develop, revise and evaluate on an ongoing basis to support clients.
-Complete an initial screening to determine eligibility and the type of assessment needed.
-Refer client to alternate services if client is ineligible for home care.
-Arrange referrals to appropriate HCN services when care requirements are emergent/urgent.
-Conduct an assessment interview, usually in the applicants home, using agency approved assessment tool.
QUALIFICATIONS:
-Bachelor of Nursing Degree
-Must have First Aide & CPR/ AED and Immunization Certificate
-Must be registered or eligible for registration and in good standing with licensing body College of Registered Nurses of Saskatchewan (CRNS).
-Experience working with First Nations and a good understanding of First Nations Health and social issues, history, traditions, and values.
-Minimum of three years related experience in public health.
