One Arrow First Nation is looking for a team member to work with clients, families and the community to promote the development of healthy communities through community-based programs, services and or activities.
DUTIES:
-Educate community on available health programs, health policies and procedures.
-Assist patients in seeking and applying for services through other resource agencies; act as an advocate to communicate the needs of patients.
-Makes referrals to providers for family meetings, Elder abuse cases and other patient concerns.
-Assist the Community Health Nurse is the delivery of community health services including immunization clinics and maternal-child health clinics by providing patient management skills.
-Assist the Mental Health Team with patient management for Harm reduction clinics.
-Delivering items such as medications, supplies, and equipment to patients home.
-Coordinates Injury prevention activities with the HeadStart and Daycare children such as bicycle/walking/car seat safety with the Primary Health Department.
QUALIFICATIONS:
-Community Health Representative Diploma and accredited Diabetes Education
-Must have First Aide & CPR/ AED certification or willing to acquire.
-Minimum of three years related experience in public health.
-Experience working with First Nations and a good understanding of First Nations Health and social issues, history, traditions, and values.
-Ability to establish and maintain cooperative working relationships.
-Ability to work independently and is highly organized
