Join our team as a Person-Centered Support Leader!

The Person-Centered Support Leader (PCSL) provides leadership, continuity, support and expertise to the person-centered support team in the daily living space of the respite program and integrated supportive living programs. The PCSL provides expertise in the delivery of the person-centered care and support while promoting person-centered decision making that honors individual choice, dignity of risk and enhanced quality of life. The PCSL supports and assist in development of individualized care and support to children, youth, and/or adults with complex needs, behavioural issues, developmental disabilities, mental health and/or other related issues.

Services
* Provide care and support to individuals with complex needs, behavioural issues, developmental disabilities, mental health challenges, and/or other related issues.
* Participate in the creation, development, support of the individuals person-centered care plan.
* Responsible to plan and coordinate schedules within the program in accordance to the person-centered care plan.
* Participate in the development and implementation of individuals activity plans.
* Teach life skills such as self care, cooking, cleaning, budgeting, and positive interactions.
* Support activities of daily living such as bathing, dressing, grooming, feeding, and toileting as needed.
* Perform health-related tasks such as medical appointment, communicate with health care professionals, and monitor health conditions.
* Assist with the administration and management of medications as prescribed by healthcare professionals.
* Resolve conflicts and respond to crisis situations using individualized behavioural support strategies including conflict resolution, crisis management, and life space interviewing.
