Montreal Lake Cree Nation is seeking a Post-Majority Worker (PMW) to support youth and young adults aged 18-26 who have aged out of the child welfare system. This role provides culturally grounded, trauma-informed services to help youth achieve housing stability, education and training, employment, and overall well-being as they transition into independent adulthood.
Key Responsibilities
- Provide case management and connect youth with housing, education, health, mental health, and cultural resources.
- Develop and support individualized transition plans and advocate for youth needs.
- Maintain accurate records (files must be able to pass compliance standards) and assist with ISC reporting requirements.
- Collaborate with the Prevention Program, Band Representative, and external service providers.
