The Youth Connect Navigator provides direct, compassionate, and supportive care to younger individuals experiencing mental health and addiction challenges. The role involves supporting youth and young adults in their journey, helping them access resources, developing coping strategies, and fostering a safe and supportive environment. This position requires collaboration with healthcare professionals, community organizations, the schools both local and urban areas and clients' support networks. Duties will include but not limited to:
-Conduct intake assessments and develop individualized support plans in collaboration with clients.
-Provide ongoing case management, helping clients identify personal goals and supporting them in achieving these through structured planning and regular check-ins.
-Lead or support group activities focused on building life skills, coping mechanisms, and resilience to aid recovery.
-Develop and deliver educational programs on mental health, addiction recovery, and wellness practices.
-Advocate on behalf of clients to ensure their rights and needs are respected within the healthcare and social service systems.
-Provide immediate support during mental health or substance use crises, employing de-escalation techniques and connecting clients to emergency resources when necessary.
-Maintain accurate, confidential records of client interactions, support plans, and progress reports in accordance with organizational and legal standards.
