Job Description: As an integral member of the Primary Health Care Team this individual will use the customer-centered approach to facilitate behavioral health changes through screening, assessments, brief interventions and education. A primary function of this individual will be to hear the customers' voice and assist them in achieving their health needs through the development of strong relationships and individualized holistic care plans. Working in a multidisciplinary environment including, but not limited to Nurse Practitioners, Physicians, Public Health Nurses, Community Outreach Workers, Nutritionists, Addiction Workers, Social Workers and other professionals and agencies within the community. The incumbent is expected to employ an effective combination of RQHR goals combined with guidance from colleagues, personal experience, post-secondary education and specialized training to assist patients in their efforts to sustain balance in all aspects of their lives.

- Bachelor of Social Work (BSW)
Experience
- Bachelor of social work and registration with SASW required.
- Experience working in partnership with the Aboriginal community.
- Experience in assessment techniques appropriate to an chronic pain population.
