Position Summary:  

The Outreach Worker will provide culturally grounded, trauma-informed support to individuals newly diagnosed with HIV, those lost to care, and those living with complex needs and co-existing conditions. This position is community-based, focusing on engaging with and meeting individuals where they are, building trust, advocating for equitable access, reducing barriers to care, and ensuring continuity of services. The Outreach Worker will walk alongside individuals in their healing journey while providing intensive case management that is flexible and tailored to meet the individuals unique needs and goals.
