LPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
LPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.
LPNs implement and evaluate the effectiveness of the care plans and make changes as required.
LPNs complete and ensure residents clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.
LPNs teach, delegate and direct unregulated care providers, providing them with coaching & mentorship.
What YOU bring
Current registration as a Licensed Practical Nurse in the applicable province
Demonstrated sound clinical knowledge
Experience working with seniors and with unregulated care providers preferred
Education and experience in gerontology, rehabilitation, community/complex/long term care an asset
Strong drive, respect and an understanding of the emotional and physical demands of the role
Focus on teamwork and customer service with the ability to build trusted relationships
Strong written and oral communication skills and critical thinking skills
Demonstrated organizational and time management skills
Computer proficiency and experience with Microsoft office applications
Current Basic Cardiac Life Support and First Aid certification required
Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
