Job Description: Under the Program Manager's direction and as part of the interdisciplinary team, the Assessor Coordinator provides screening, triage, assessment, treatment and case coordination services to a community mental health and addictions population. The Community Outreach & Support Team provides crisis intervention, support, assessment and follow-up plans to clients. The team assists in stabilization and/or transition of acute patients from hospital/detox to community or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those in crisis. Follow-up may include brief monitoring/support, referral to appropriate community organizations/support networks or collaboration with supportive organizations. Services are provided through extended hours (days, evenings and weekends).

Human Resources Exemption: No
Education
- Recognized degree/diploma in nursing; degree in social work; or a degree in addiction counselling.
Experience
- One year (postgraduate) recognized experience in working within community mental health and addiction services.
Knowledge, Skills and Abilities
- Knowledge of a variety of mental health disorders and addiction and harm reduction strategies required to enable clients to sustain stability.
