0.8 FTE. Monday to Saturday. All sites. Flexible hours may be required which may include working some evenings and weekends as scheduled.
To promote the mental, emotional and social health and well-being of Saskatoon Community Clinic patients. To do this through assessing client needs, developing individualized service plans, coordinating resources, and monitoring progress to ensure effective delivery of support services in collaboration with the mental health team. This role services as the primary point of contact for clients, advocating on their behalf and facilitating communication among service providers, families and community agencies.
DUTIES:
Takes responsibility as primary case manager for clients.
Interviews clients and assesses personal, family, social, and employment functioning relevant to the problems and symptoms presented. Assesses social and other environmental conditions.
Determines a service plan and facilitates it through: system navigation, referral, group treatment, connecting clients to cultural services and resources, environmental change and counselling. Monitors and follows-up on client progress.
Works in collaboration with other health care workers in planning and service provision.
Refers clients to appropriate community agencies and services for assistance in improving personal, social and vocational functioning and for practical help.
Provides crisis and ongoing counselling.
