THE POSITION:

The Health Transformation Coordinator is responsible for leading the development and management of Grants program and operations budget.  Under the direction of the Director of Health, the Health Transformation Coordinator, provides team leadership for developing and implementing strategic direction and priorities for an impactful Grant program.
DUTIES AND RESPONSIBILITIES:
Leadership
    Demonstrates consistent commitment to values of equity, Health, and social justice through personal and organizational leadership practices. Supervises and supports growth, development and performance of a small team of professional staff and consultants who perform a range of proposals and community engagement activities.
Proposal making
    Provides team leadership for developing and implementing strategic direction and priorities for an impactful proposal program.
Community Engagement
    Works proactively to ensure the Foundation is engaging people and communities whose voices and experience have been under-represented and/or excluded in land use planning and decision-making.
