Job Description: Community Care is a delivery model that utilizes integrated, interdisciplinary teams of health professionals and community engagement/partnerships to provide high quality service as close to home as possible, at the most appropriate time, by the most appropriate person. Under the direction of the Manager, the Registered Nurse carries major responsibilities in providing nursing services in the home and community health centre; collaborates as a member of an inter-disciplinary team in assessing needs, setting priorities for care and developing standards for care; participates in educational programs and performance evaluation process; promotes good public relations and participates, as required, in studies and research projects within the Community Health Centre and Home Care Program.
