Job Description: The Specialist supports all Continuing Care programs, including Long Term Care, Transitional Care, Rehabilitation, Geriatric Services, and Program Development. They perform key functions to advance strategic and operational goals, ensuring high-quality care and standards. The role requires critical thinking, systems-level awareness, and the ability to act as a change agent for quality and program initiatives. The Specialist helps plan, recommend, and implement standards, processes, and safety work, supporting teams of in- and out-of-scope staff in projects, policy development, accreditation, clinical procedures, and service changes. They collaborate with affiliated providers and SHA teams across Continuing Care and work with provincial partners such as Clinical Standards, Quality and Safety, Digital Health, and Integrated Health Services.

Experience
- 3-5 years experience in Long Term Care, Rehabilitation or Geriatric Services
Education
- Undergraduate degree or diploma in a health related field
Licenses
- Valid Class 5 driver's license

- Licensed, and in good standing, with regulatory body, if applicable
- Undergraduate degree or an equivalent combination of education and experience
