Job Description: The Assessor Coordinator is one of 3 positions that will report to the Home Care Manager NW5 Wilkie, Unity and Macklin area and work in partnership with the Nurse B  Home Care Nurse Unit Coordinator. The Assessor Coordinator is accountable for the day to day assessing, planning and coordinating referrals and services for home care referrals and hospital-based clients. The Assessor Coordinator ensures the transition from hospital to discharge occurs in as timely a manner as possible.

Experience
- Minimum of 1 year nursing experience in Acute or Home Care Setting
Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Registered Psychiatric Nurse license is acceptable
- Practical Nursing diploma with 4 years experience is acceptable
- Bachelor of Psychiatric Nursing is acceptable
- Psychiatric Nursing diploma is acceptable
- Nursing diploma is acceptable
- Current registration w/ Registered Psych. Nursing Assoc. of SK (RPNAS) required
Knowledge and Abilities
- Ability to work independently and as a leader of a multidisciplinary team
- Ability to assist and develop policies and procedures according to best practice
- Makes judgements on the basis of critical thinking and problem solving criteria
