Job Description: The Nurse Navigator, Kidney Health plays a crucial role in supporting patients navigating dialysis treatment and vascular access options. This position is responsible for promoting goals related to independent dialysis and body access, collaboration, with medical and inter-professional teams, inpatient teams, Kidney Health partners and other stakeholders. The Navigator facilitates a seamless patient journey during transitions, ensuring timely education on dialysis treatment options and coordinating services for optimal patient wellbeing. The Nurse Navigator works closely with key stakeholders to ensure patients receive comprehensive education on all aspects of dialysis treatment options enabling the patient to make an informed choice regarding dialysis treatment and vascular access.

Human Resources Exemption: No

Experience
- Successful Completion of "Care of the Patient with Kidney Disease" Course
- Five (5) years experience in a related area
- Experience in program development
Education
- Serves as an educator and resource person for renal patients.
- Creates and implements standardized patient education and assessment tools
- Assesses patients with End-Stage Renal Disease (ESRD) to ensure patients can make an informed dialysis treatment and access choice decision.
- Assesses the patients understanding of renal disease and treatment options at time of admission to Hemodialysis Program.
