Sanctum Care Group  Prenatal Outreach Resource Team (PORT)

Sanctum Care Group is seeking Case Managers to join the new Prenatal Outreach Resource Team (PORT). This team provides community-based, wrap-around support to expectant women in Saskatoon who are at risk of having their baby apprehended. The goal is to promote healthy pregnancies and help women successfully parent if they choose.

PORT Case Managers work closely with community partners to coordinate psychosocial, mental health, addictions, and physical health services. Team members address barriers as they arise and keep the wellbeing of both the mother and unborn baby at the center of their work.

Successful applicants demonstrate strong commitment to trauma-informed care, harm reduction, and meeting people where they are at. All interactions must be respectful, non-judgmental, and aligned with Sanctum Care Groups mission and values. Knowledge of community resources and experience working with diverse populations are essential.

We are seeking strong problem solvers, advocates, communicators, and team players. The role is highly community-based and requires comfort working independently and in the field. A vehicle is required.

Key Responsibilities
-Provide case management to prenatal and postnatal women
-Maintain accurate documentation and charting
-Engage clients in community settings
-Work independently and as part of a team
-Collaborate with agencies and provide referrals

Advocate for clients and problem solve in fast-paced situations

Qualifications
-Bachelors degree in Social Work, Human Services, or related field (or equivalent experience)
-Valid drivers license
-Access to a reliable vehicle
