Education
- Must have a current practicing registration with their professional association
- Degree in Human Services/Health Sciences or related field
- Recognized degree in nursing , or a social work degree.
Knowledge and Abilities
- Substitute Decision Maker Act, Community Care Branch Ministry of Health
- Knowledge of theories, concepts, intervention strategies for case mgmt practice
- Client confidentiality and the Health Information Protection Act
- Knowledge of the Truth and Reconciliation Commission Calls to Action
- Personal Care Home Act & Regulations, Saskatchewan Health Authority policies.
- Relevant data, assessment, plan of care/interventions, updates, discharge summ.
- Effective interpersonal relationship skills
- Knowledge of First Nations and Metis history in Saskatchewan
- Cost conscious, quality driven care planning and service delivery
- Highly developed written and oral communication skills
- Openness to teaching and providing educational opportunities.
- Ability to adapt to new techniques and best practices
- Advocacy, negotiation, crisis management and conflict resolution
- Mental Health Act, Child and Family Services Act, Health Care Directive
