Description

The Home and Community Care Coordinator will provide supervision and direction to the Home and Community Care Program. The main role of this position is to ensure that effective, efficient service is available to community members who require assistance to continue living independently. The HCC Coordinator will report to the Director of Health.
The HCC Coordinator is also responsible for program development, quality assurance and staff supervision.
