Develops and implement Individualized Support Plans that assist young adults with varying abilities to prepare for and secure employment and achieve long term success in the work environment. As well as, develop and maintain positive working relationships with employers and support agencies throughout the region.
Duties include:
- Recruit and select clients, conduct intakes and develop strength-based action plans based on clients unique circumstances and goals, and help connect under-served youth to necessary wrap-around supports
- Identify clients need for and promote to employers the use of work experience, job coaching, job carving, job accommodations, training on the job, productivity support, and work assessments.
- Provide case management services to participants of the program
- Create/Review/Update project curriculum, develop teaching methodology, organize teaching material, and create power point presentations
- Deliver one-on-one and group-based vocational life skills sessions in communities throughout the region
- Conduct outreach to develop new strength-based partnerships/liaise with existing strength-based partners in order to gain a deeper understanding of barriers and other intersectionality factors impacting individuals with disabilities (including those from equity deserving groups) in preparation to support clients and employers in creating conditions for successful work placements
Qualifications:
Education:
- University degree or equivalent professional designation with experience
- Valid Class 5 Driver's Licence and a reliable vehicle to transport
