General Description
The LPN/RN in the HCC Program provides direct nursing care, under the direction/supervision of a HCC RN/Community Health Nurse (RN).
Duties Relating to Program Delivery
The duties within this position relate to providing direct nursing care to clients under the team lead of a HCC RN. These duties include:
Receives the assignment from the HCC RN and demonstrates an understanding of the assignment.
Performs the nursing skill required to meet the assigned task in a safe competent manner.
Reports any changes in the client condition/situation to the HCC RN.
Documents the results of the assigned tasks.
Involves client/family in performing tasks, whenever possible.
Recommends, to the HCC RN, revisions to the client/family-centered care plan as appropriate
Duties Relating to Accountability
The LPN/RN is accountable for reporting, recording, working within program guidelines and managing the workload of the assigned tasks. Duties in this area may include:
Reporting and concerns about the clients safety or health risks to the HCC RN.
Reporting any changes in the clients condition/situation to the HCC RN.
Participating in case review conferences.
Carrying out assigned tasks.
Maintaining Confidentiality
Reinforcing program mission statement and philosophy to client, family and community.
Notifies HCC RN when unable to complete assigned task.
Submitting claims and time sheets as required.
Submitting monthly statistics/reports as required
Working within their scope of practice
