Job Description: Working collaboratively with a number of regulated and unregulated providers, including First Nations' community-based health teams and health leaders, the primary responsibility of the Social Worker - BSW - Senior will provide assertive outreach case management services in the community to urban and on-reserve First Nations population of primarily women of child-bearing age and their families. The Social Worker - BSW - Senior will serve as a recognized resource for other professionals' health and social services navigation, providing a range of screening, triage, assessment and case coordination and psychosocial case management supports, coordinating services and supports and managing crisis as relevant. The Social Worker - BSW - Senior, will identify, collect, analyze data and provide reports on program outcomes, including recommendations to enhance seamless access to equitable and culturally responsive care.
Education
- Bachelor's degree in social work
- Registered or eligible for registration with Saskatchewan Association of Social Workers (SASW).
Experience
- Minimum two (2) years recent recognized clinical experience in a community setting working with women and families.
- Clinical experience working collaboratively with First Nations women and families is an asset.
