Job Description: Under the direction of the Manager of the Pediatric Inpatient Department, the Nurse Coordinator is responsible for the coordination of care and ensuring continuity of care for the pediatric patient with end stage renal failure requiring renal replacement therapy and/or transplant. The incumbent will be responsible for assisting with the development and ongoing coordination of the provincial hemodialysis and kidney transplant program. They must work cross functionally within the portfolio and across the organization to support operations and services locally and provincially. Assisting the patient and family to coordinate and access timely, appropriate care through the health care system efficiently. The incumbent will develop and foster relationships with other health care providers including community partners and agencies to gain familiarity with and mobilize resources for the patient and family. Additional responsibilities include quality improvement and research initiatives.

Human Resources Exemption: No

Experience
- 3 years experience caring for children with chronic kidney disease or failure
- Experience in hemodialysis or continuous renal replacement therapy is an asset
- Exp. related to leadership, patient education, program development & evaluation.
Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Psychiatric Nursing diploma is acceptable
