Description

Community Coordinator
Sanctum 1.5 is a non-profit organization providing transitional care and support to pre- and postnatal women living with or at high risk for HIV/AIDS who may also experience mental health challenges, addictions, poverty, and homelessness.

The Community Coordinator provides case management and support to graduates of the Sanctum 1.5 program for up to 12 months post-discharge. This role works collaboratively with community partners to coordinate mental health, addictions, medical, and psychosocial supports. The Coordinator supports clients in achieving positive maternal and infant outcomes, strengthening life and parenting skills, and navigating community systems. Therapeutic counselling and in-home or community-based support are provided to clients facing complex challenges that may impact parenting success.
This is a temporary position to cover a leave of absence.

Qualifications & Knowledge
Degree in Social Work, Human Services, or a combination of education and relevant experience
Cultural sensitivity and strong communication skills
Knowledge of harm reduction, trauma- and violence-informed care, addiction and recovery
Understanding of HIV in a family context, neonatal abstinence syndrome, and child welfare involvement
Familiarity with the Child Protection Act and community support services
Ability to advocate, problem-solve, and work in a fast-paced environment
Strong decision-making, teamwork, and relationship-building skills
Ability to work flexible/field hours
Valid drivers licence and access to a vehicle

Maintain accurate charting and documentation
Connect clients with community resources and referrals
Support safety, crisis intervention, and successful parenting outcomes
Track, measure, and document client outcomes
