Description

Position Overview:
We are seeking a dedicated Youth Transitions Consultant to work with high school youth with disabilities, providing guidance and tools for transitioning into higher education or the workforce. The ideal candidate is passionate about youth development and excels in building supportive relationships. This position serves Estevan, Weyburn and surrounding areas.
Key Responsibilities:
Individual Support:
Conduct one-on-one sessions to assess needs and goals.
Develop personalized transition plans.
Provide ongoing mentorship and support.
Skill Building:
Facilitate workshops on communication, time management, and problem-solving.
Assist with building resumes and cover letters.
Guide job search strategies and interview preparation.
Teach financial literacy, understanding pay stubs, apartment inspections, and rental agreements.
Resource Coordination:
Connect youth with educational programs, vocational training, and job opportunities.
Maintain relationships with schools, colleges, employers, and community organizations.
Monitoring and Evaluation:
Track and document participant progress.
Evaluate and adjust transition plans as needed.
Qualifications:
Experience working with diverse youth populations.
Strong understanding of challenges faced by youth with disabilities.
Excellent communication, organizational, and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in Microsoft Office.
Reliable transportation, valid driver's license, and clear criminal record check.
