Job Description: Provides assessment, care planning, client advocacy, and coordinating services and monitoring care needs. Participates in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community. Collects, distributes and documents client information, facilitating client conferences and assisting with active treatment phase. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Human Resources Exemption: No

Education- Bachelor of Science in Nursing (BScN)
Licenses- Basic Life Support Certificate (BLS)
- Valid Class 5 driver's license
Other Education and Training- Bachelor of Social Work is acceptable
- Occupational Health Nursing certificate is acceptable
Competencies - Basic - Computer skills
Other Information- Registered/eligible for registration with the appropriate licensing body
- Ponteix is located approximately 85 km south of Swift Current
