Job Description: The Case Manager/Assessor is responsible for providing assessment, care planning, client advocacy, and coordination or services and monitoring of care needs. As a team member, the Case Manager will participate in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Human Resources Exemption: No

Experience- Clinical experience in health care
Education- Bachelor of Social Work (BSW)
Licenses- Basic Life Support Certificate (BLS)
- Valid Class 5 driver's license
Other Education and Training- Bachelor Degree in a health related field
Knowledge and Abilities- Knowledge in home care case management
Other Information- Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
