Description

PORT (Prenatal Outreach Resource Team) is seeking a Registered Nurse to add to their teams. PORT provides community-based support and resources to pre and postnatal women and their infants, who are or were at risk of having their infant apprehended at birth.

The RN works with existing wrap around care and support offered by Sanctum Care Group through a harm reduction and trauma informed lens. The RN will coordinate medical care and follow up for both women and infants related to but not limited to pre and postnatal care, syphilis and HIV testing & treatment, STI/STBBI monitoring, Opioid Agonist Therapy, medication adherence, fielding medical concerns, consulting with Pediatrics, Infectious Disease, facilitating medical appointments, infant care, breastfeeding education, mood assessments and referrals to Primary Care.

This position will be actively engaging clients in the community. The RN must have the ability to communicate and maintain effective working relationships with clients from diverse social, economic, and cultural backgrounds, and maintain an effective working relationship with other members of team, health care providers and community agencies.

Experience in public health, pre and postnatal care, pediatric care and or mental health and addiction nursing is an asset.
Maintain accurate client charts
Travel required
Strong knowledge of service providers within the areas we serve
Understanding of the issues related to communicable disease, poverty, addiction, trauma and mental health
Understanding of harm reduction and its role in providing opportunistic care
Understanding of trauma informed care and its role in providing culturally sensitive and appropriate interventions for populations who have been exposed to trauma
Willingness to work with a team and independently
Bachelor Degree in Nursing & Valid RN License
