Description

Sanctum 1.5 is hiring a Full-Time Temporary Care Coordinator
Main Activities
Work with pre and postnatal women living with or at high risk for HIV/AIDS who may be affected by mental illness addictions poverty homelessness
Coordination of psycho social components of individual to maximize success in achieving favorable resident/ infant outcomes enhancing life and parenting skill coordinating housing for transition into the community
Work with residents on individual goals and care plans aimed at recovery life and parenting skills  
Provide therapeutic counselling to clients who may present with multiple issues affecting success in parenting their child
Ensure outcomes are recorded measured and documented
Assist clients as needed moving into supported housing which may include accessing rental assistance money security deposit donated furnishings coordinating with movers and coordinating transportation
Assist clients in overcoming barriers related to successful parenting of infant housing and providing crisis intervention/prevention as needed to maintain housing
Qualifications
Cultural sensitivity of First Nations issues
Extensive knowledge of community resources with an emphasis on housing services and how to access and advocate for these services on behalf of residents
Ability to communicate and maintain an effective working relationship with clients from diverse socio- economic and cultural backgrounds.
An understanding of the issues related to HIV harm reduction in family context, vertical transmission and care of infants born to HIV positive women
Understanding of neonatal abstinence syndrome
understanding of the current issues regarding child welfare involvement in vulnerable populations  
Conflict resolution and interpersonal skills
Background working in Mental Health and Addictions
Degree in social work addictions or related field
