Job Description: Reporting to the Manager or designate, the Social Worker provides a range of screening, triage, assessment, treatment, and case coordination services to primarily adult community mental health and addiction population. The Social Worker provides services as a member of a multidisciplinary team. The incumbent will work collaboratively with community groups, agencies, and other professionals within the community.

Education
- Bachelor of Social Work (BSW)
Knowledge and Abilities
- Expert knowledge and advanced skill in a variety of treatment modalities
- Knowledge in health promotion and prevention strategies
- Ability to complete case management assessments, follow-up and treatment
- Committed to delivering on patient and family centred care
- Knowledge in local human service organizations, resources, referral procedures
- Strong interpersonal skills
- Systems Theory/Family Systems Theory, strength-based model, crisis intervention
- Demonstrates strong communication and organizational skills
- Knowledge in individual assessment and counselling skills
- Committment to self evaluation, professional development and ongoing self care
- Ability to work cooperatively in an integrated interdisciplinary team
- Ability to work with a variety of individuals presenting with complex issues
- Understanding social determinants of health for program development & delivery
