The Outreach Worker serves as the first point of contact for individuals referred to the Thread Program, providing initial assessments, early support, and regular check-ins. This role collaborates closely with Counsellors and the Community Connector to ensure participants are safely engaged, supported, and connected to appropriate resources in the community.
Key Responsibilities:
Respond to referrals and conduct initial participant assessments.
Provide ongoing check-ins with participants and liaise with Counsellors.
Collaborate with the Community Connector to link participants to resources.
Maintain accurate records and participate in team meetings to monitor progress.
Foster a safe, supportive, and respectful environment for all participants.
Qualifications:
Background in Social Work, Human Services, Psychology, or related field.
Experience in outreach, crisis intervention, or community support.
Strong communication, organizational, and interpersonal skills.
Requirements:
Valid drivers license and reliable access to a vehicle.
Satisfactory Criminal Record Check including Vulnerable Sector Search.
Commitment to maintaining confidentiality, professionalism, and a trauma-informed approach.
