Job Description: Accountable for overall coordination and support of primary health and chronic disease management activities, including multiple site operations, linking with internal and external partners and joint supervision of designated staff. The incumbent is also accountable for assisting in and contributing to the operationalization of the Primary Health and Chronic Disease Management Strategic Plan as approved by the Senior Leadership Team, in accordance with the vision, mission, values and strategic direction of the Saskatchewan Health Authority and St. Paul's Hospital. There are numerous locations of primary health and chronic disease management services in the region. Special attention must be given to the complexity of delivering services in several different sites and locations throughout the region.
