Description

0.8-1.0 FTE.  Monday to Saturday. All sites. Flexible hours may be required, which may include working some evenings and weekends as scheduled.  

CORE FUNCTION:

To promote the mental, emotional and social health and well-being of Community Clinic clients and their families. To do this through counselling, referral, education, advocacy, community action, program leadership, and connecting clients to cultural services and resources.

DUTIES:

Provides crisis and brief counselling to Community Clinic clients.

Takes responsibility as primary case manager for clients.

Interviews clients and assesses personal, family, social, and employment functioning relevant to the problems and symptoms presented. Assesses social and other environmental conditions.

Determines a service plan and facilitates it through: case management and brief counselling, referral, group treatment, connecting clients to cultural services and resources and/or environmental change. Monitors and follows-up on client progress.

Treats clients emotional, psychological, and relationship difficulties using a range of counselling models and methods.

Works in collaboration with other health care workers in planning and service provision.

Refers clients to appropriate community agencies and services for assistance in improving personal, social and vocational functioning and for practical help.

Seeks consultation and supervision as necessary.
