Through funding made possible by Saskatchewan Seniors Mechanism, the City of Swift Current has entered into an agreement to be a pilot initiative for Social Prescribing in older adults. This 18-month project funds a half-time contract to implement social prescribing for older adults.
The Community Connector accepts referrals from health care providers or other sources to connect one-on-one with older adults to empower them to take control of their health and wellbeing through a holistic
approach by linking them with community groups and services for practical and emotional support.
This role also helps strengthen community capacity by working collaboratively with community groups, SHA and health partners to ensure local assessments are strong, inclusive, and sustainable.
