Job Description: The Assessor Coordinator  Degree - Senior is a supervisory, team leader position in Primary Health Care, reporting directly to the Manager. The position will provide clinical supervision for Assessor Coordinators and Social Workers on the team in the functions of case management practice. The position requires the use of high order knowledge, clinical expertise and strong interpersonal skills to coach, mentor, orientate and provide education to the team members. Drawing on standards of practice for case management; professional codes of ethics and standards of practice in health care; program policy and procedures; and, relevant legislation, the incumbent will ensure that the team provides high quality client service and professional standards of practice are maintained.

Education
- Recognized degree in nursing or social work. Must have a current practicing registration with Saskatchewan Association of Social Workers or the College of Registered Nurses of Saskatchewan.
Experience
- Minimum two years related clinical experience with demonstrated leadership ability and case management.
Knowledge, Skills and Abilities
- Advanced clinical skills, knowledge, and abilities required for competent case management practice.
