Key Responsibilities

Deliver home care nursing services and health education

Monitor client health and maintain accurate records

Support community health programs and reporting

Collaborate with healthcare providers and families

Participate in case management and care planning

Qualifications

Registered Nurse (RN) in Saskatchewan

Experience in home care or community health preferred

Strong assessment and case management skills

Valid drivers license
